Why is BBL so Dangerous

To understand why is BBL so dangerous? I thought I’d try to explain some of the science behind the headlines since Brazilian Butt Lift surgery, also known as fat grafting to the buttocks, has been in the news once more. BBL has the highest mortality rate compared to other cosmetic procedures. 

According to estimates, 1 in every 3,000 to 5,000 procedures is performed. The British Association of Aesthetic Plastic Surgeons decided its members should not perform the procedure.

All of this concerns the inferior gluteal vein (IGV), an anatomical structure. It is a large vein in the buttocks where BBL work is done. In every post-mortem of BBL death victims, fat has been found injected into this vein resulting in a typical fatal fat embolism. 

Is it, therefore, a risky procedure just because of that vein? First and foremost, the IGV differs slightly from other body veins. IGV is a fat, floppy vein, and it is fixed in contrast to other veins, which can move out of the way when you approach them.

IGV is fixed in place, passing under a structure of your pelvis known as the greater sciatic notch. This indicates that surgeons cannot avoid it while doing the BBL procedure. 

The more concerning fact is that when surgeons perform fat grafting, they may tear IVG without actually puncturing it. It gets torn simply because of vigorous wobbling of the bottom. Therefore, the IGV can be torn without being punctured.

Like those of other veins, the vein’s valves only allow blood to flow in one direction, so when the surrounding muscles compress the vein, blood moves toward the heart. A negative pressure (or suction) effect is created by this one-directional movement, allowing fat or anything else to be sucked into the vein’s vicinity through the tear.

Therefore, we have a structure that can be torn during the BBL procedure without the surgeon’s knowledge, and this raptured vein can suck fat from the surrounding areas.

The surgeon could not believe that they had grafted fat below the muscle found in every post-mortem. They believed that they were grafting fat always in or above the muscle.

Therefore, if the IGV had ruptured and suctioned fat from another location, and if that is the case, then this problem could happen to anyone; even if a good technique is used, it could be a hazardous procedure. 

A UK Task Force has researched the safety of BBL, determining whether it can be performed safely at all, and measures will be taken to prevent the tragic deaths that have already occurred and even occur again. It should be suspended for the time being until finding a viable solution.
